Objective Pelvic tilt and lumbar lordosis angles are key postural parameters routinely evaluated in orthopedic assessments for low back pain. This study aimed to compare pelvic tilt angle (PTA) and lumbar lordosis angle (LLA) between patients with chronic non-specific low back pain (CNSLBP) and healthy asymptomatic peers during hip extension and knee flexion maneuvers.
Materials & Methods This cross-sectional comparative study involved 124 subjects aged 20-45 from Babol, Iran, including 62 patients with CNSLBP and 62 asymptomatic controls. A flexible ruler was used to measure the LLA in the prone position, and an inclinometer measured PTA in the standing position under three specific testing conditions: relaxed, immediately after active hip extension, and immediately after active knee flexion. Data analysis was conducted in SPSS software, version 23, with a significance level set at 0.05.
Results No significant differences were found between the case and control groups in the mean prone LLA or mean standing PTA under any measurement conditions (P>0.05). A weak positive and significant correlation was found only between pain intensity and standing PTA after hip extension (P=0.022). Within-group analysis revealed that LLA values after hip extension and knee flexion were significantly greater than that under the relaxed prone condition (P<0.001), and PTA after hip extension was significantly higher compared to PTA in the relaxed standing position (P<0.05) in both groups.
Conclusion LLA and PTA in relaxed position or following active hip extension and knee flexion tests are not significantly different between individuals with CNSLBP and asymptomatic controls. These parameters may therefore have limited clinical utility in assessing patients with CNSLBP.
