هدف اختلال در کنترل تعادل به عنوان عامل اصلی اختلال در کارکرد کودکان فلج مغزی است. هدف از مطالعه حاضر بررسی تأثیر تحریک شنیداری ناملایم بر تعادل ایستای کودکان فلج مغزی اسپاستیک ۵ تا ۱۲ سال تهران است.
روش بررسی در این مطالعه توصیفی تحلیلی ۲۰ کودک فلج مغزی اسپاستیک (۱۲ دختر و ۸ پسر با میانگین سنی ۲/۰۳±۷/۱۰ سال) شرکت کردند. نمونه‌گیری به شکل آسان و از جامعه در دسترس صورت گرفت. در این مطالعه کودکانی با تشخیص فلج مغزی اسپاستیک توسط متخصص مغز و اعصاب، سن تقویمی ۵ تا ۱۲ سال، قرارگرفتن در سطح I یا II بر اساس سیستم طبقه‌بندی حرکات درشت، توانایی فهم دستورات شفاهی، توانایی ایستادن مستقل حداقل برای ۳۰ ثانیه، قوام عضلانی ۱ تا ۳ در ناحیه مچ پا بر اساس معیار تعدیل‌یافته آشورث، وارد شدند. این کودکان با بروز اختلال بینایی یا شنوایی و حمله تشنجی از مطالعه خارج می‌شدند. ابزارهای استفاده‌شده در این مطالعه شامل صفحه نیرو، پرسش‌نامه اسپارکل و معیار تعدیل‌یافته آشورث بودند. صفحه نیرو، برای بررسی تعادل ایستا، پرسش‌نامه اسپارکل برای تعیین سطح عملکرد هوشی و معیار تعدیل‌یافته آشورث برای تعیین میزان شدت تون عضلانی در کودکان فلج مغزی استفاده شدند. اطلاعات مربوط به تغییرات مرکز توده بدن با دستگاه صفحه نیرو مدل ۹۲۸۶A کیستلر ساخت کشور سوئیس تحت فرکانس ۱۰۰ هرتز و نرم‌افزار ۱۲-۰-Bioware ۴ اندازه‌گیری شد. تمامی شرکت‌کنندگان در وضعیت ایستاده و با چشم باز ارزیابی شدند. هر کودک با پای برهنه درحالی‌که دست‌هایش کنار بدنش بود، روی نقاط نشان‌دارشده به مدت ۳۰ ثانیه روی صفحه نیرو می‌ایستاد که یک بار با پوشش سخت و یکبار با پوشش نرم فرش شده بود و به عکس منظره‌ای نگاه می‌کرد که در فاصله دو متری و دقیقاً روبه‌روی او قرار داشت. سپس تحریک شنیداری ناملایم با دو شدت متفاوت (بالا و پایین) از پشت سر کودک پخش می‌شد. مدت زمان انجام هر تکلیف ۳۰ ثانیه بود و سه بار تکرار می‌شد. چهار تکلیف شامل ایستادن روی سطح نرم با تحریک شنیداری ناملایم شدت بالا؛ ایستادن روی سطح نرم با تحریک شنیداری ناملایم شدت پایین؛ ایستادن روی سطح سخت با تحریک شنیداری ناملایم شدت بالا؛ و ایستادن روی سطح سخت با تحریک شنیداری ناملایم شدت پایین بود.کودک اجازه داشت در فواصل مناسب (بعد از هر سه بار تکرار) بین تکالیف استراحت کند. داده‌ها برای هر کودک سه بار، هر بار به مدت ۳۰ ثانیه و با فرکانس ۱۰۰ هرتز ثبت شدند. داده‌ها در نرم‌افزار متلب نسخه R۲۰۱۰a تحت فیلتر پایین‌گذر پردازش شدند. سپس برای مقایسه تأثیر تحریک شنیداری ناملایم در شدت‌های بالا و پایین بر سطوح نرم و سخت با چشم باز و در حالت ایستاده بدون حرکت، سرعت جابه‌جایی و تصویر سطح پایه در صفحه قدامی خلفی ابتدا نرمالیته داده‌ها با روش شاپیرو ویلک محاسبه شد. سپس به روش آماری t زوجی در نرم‌افزار SPSS نسخه ۱۶ تجزیه‌وتحلیل شد. سطح معناداری برای تجزیه‌وتحلیل داده‌ها P<۰/۰۵ در نظر گرفته شد.
یافته‌ها تأثیر تحریک شنیداری ناملایم با شدت بالا در هر دو حالت ایستادن روی سطح نرم و سخت بر روی سرعت جابه‌جایی در صفحه قدامی خلفی و تصویر سطح پایه در صفحه قدامی خلفی معنادار بود (P<۰/۰۰۱)، اما تحریک شنیداری ناملایم با شدت پایین در هر دو حالت بر روی تصویر سطح پایه در صفحه قدامی خلفی (۰/۳۸۰=P) و بر سرعت جابه‌جایی در صفحه قدامی خلفی (۰/۷۲۲=P) تأثیر معناداری بر کنترل تعادل ایستا نداشت.
نتیجه‌گیری بر اساس نتایج حاصل از این مطالعه، تحریک شنیداری ناملایم با شدت بالا روی سطوح سخت و نرم صفحه نیرو در وضعیت چشم باز در میزان سرعت و تصویر سطح پایه صفحه قدامی خلفی و در برهم خوردن تعادل ایستای کودکان فلج مغزی اسپاستیک دوطرفه تأثیر دارد. بنابراین می‌توان در طراحی برنامه‌های تعادلی تحریکات شنیداری با شدت بالا را بر سطوحی با درجات متفاوت سختی در نظر گرفت.
