Background and aims: The carpal tunnel syndrome (CTS) is one of the most common problems in people referring to hand rehabilitation clinics. The aim of this study was to comparing the effects of custom-molded leathern cockup and prefabricated cockup on sensory-motor function of patients with mild-to-moderate CTS.
Methods: This was a quasi-experimental study on 34 housewives with mild-to-moderate CTS which divided in two groups of custom leather cock-up (with involvement of 25 hands) and prefabricated cock-up (with involvement of 24 hands). Sensory-motor functions of participants in both groups were evaluated before the initiation splinting, after six weeks of splinting, and three weeks after discontinuation of splints using Boston, Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaires and 2-point discrimination (2PD) test. The data were analyzed by Mixed between-within subject ANOVA and Post-hoc tests.
Result: six weeks after splinting, the median pain was higher in the leather cock-up group (p = 0.01). Three weeks after discontinuation of splints, the median pain was equal in both groups (p=0.23). Results showed the improvement in pain, range of motion, 2PD, strength of the grip and pinch, quality of life (QoL) and hand function were similar in both groups. However, except QoL parameters and results of the DASH questionnaire that in prefabricated cock-up was significantly better than in custom cock-up (p=0.02, and p<0.001, respectively), there were no significant differences in other parameters between two groups.
Conclusion: Our findings show that both pre-fabricated and leather splints improved clinical outcomes, although the pre-fabricated splint showed superior results in some parameters.
