fa بررسی تغییرات شیب تابع رشد دامنه پتانسیل عمل مرکّب الکتریکی طیّ سه ماه بعد از دریافت پردازشگر گفتار در کودکان کاشت حلزون‌شده عمومى General پژوهشی Original <p style="text-align: justify"><strong>هدف:</strong> در تله متری پاسخ عصبی، تحریک و ثبت پاسخ عصب شنوایی توسط الکترودهای درون حلزونی پروتز کاشت حلزون صورت می‌پذیرد. در این جا تحریک الکتریکی توسط یک الکترود به عصب شنوایی ارائه و پاسخ حاصله که پتانسیل عمل مرکب الکتریکی نامیده می‌شود، توسط یکی از الکترودهای مجاور ثبت می‌گردد. مهم‌ترین کاربردهای بالینی این آزمون عبارتند از ارزیابی و پایش پاسخ‌های عصب شنوایی در حین و بعد از عمل جراحی و کمک به تنظیم اولیه پردازشگر گفتار. هدف این مطالعه، ارزیابی تغییرات شیب تابع رشد دامنه این پتانسیل طی سه ماه بعد از دریافت پردازشگر گفتار در کودکان کاشت حلزون شده بود.<br> <strong>روش بررسی:</strong> در این مطالعه طولی با استفاده از آزمون فریدمن، تغییرات شیب تابع رشد دامنه این پتانسیل طی چهار جلسه پس از دریافت پردازشگر گفتار و با فواصل زمانی تقریباً یک ماهه، در الکترودهای اول، هشتم، پانزدهم و بیستم در تمامی کودکانی که در تابستان و پاییز سال 1386 در بیمارستان‌های امیر اعلم و حضرت رسول اکرم(ص) تهران تحت عمل جراحی کاشت حلزون قرار گرفته بودند، بررسی شد.<br> <strong>یافته‌ها:</strong> میانگین شیب‌ها، در ادامه کار بیش از جلسه اول بوده و مقایسه بین الکترودی نتایج حاکی از کاهش معنی دار میانگین شیب تابع رشد دامنه در الکترودهای رأسی‌تر نسبت به الکترود اول بود (۰/۰۵>P).<br> <strong>نتیجه‌گیری:</strong> ثبات نتایج حاصله می‌تواند باعث اطمینان خاطر بیشتر درمانگر جهت تنظیم پردازشگر گفتار برای مدتی نسبتاً طولانی گردد. هم چنین نتایج بهتر الکترودهای رأسی‌تر نیز می‌توانند موجب ارائه و اعمال یک راهکار کدگذاری مؤثرتر گردند.</p> <p dir="ltr" style="text-align: justify"><strong>Objective: </strong>In neural response telemetry, intracochlear electrodes stimulate the auditory nerve and record the neural responses. The electrical stimulation is sent to the auditory nerve by an electrode and the resulted response, called electrically evoked compound action potential, is recorded by an adjacent electrode. The most important clinical applications of this test are evaluation and monitoring the intra and postoperative responses of auditory nerve and help to primary setting of speech processor. The aim of this study was evaluating the potential&#39s slope of amplitude growth function changes three monthes after receiving the devise in pediatric cochlear implant recipients.</p> <p dir="ltr" style="text-align: justify"><strong>Materials & Methods:</strong> This longitudinal study evaluated the potentials&#39 slope of amplitude growth function changes in four given electrodes in four sessions after receiving the devise by approximately one month&#39s intervals in all of the children who implanted in Amir Alam and Hazrat-e-Rasoul hospitals in 2007, July to December. Friedman test was used to analyse the results.</p> <p dir="ltr" style="text-align: justify"><strong>Results:</strong> Electrically evoked compound action potential&#39s mean slope of each electrode was more in later sessions relative to first session, while there was significant difference between the 1st and the other electrodes’ responses in every session (P<0.05).</p> <p dir="ltr" style="text-align: justify"><strong>Conclusion: </strong>The reliabiliy of the responses results in more assurance of clinician to fit the speech processor for along time.
