fa بررسی علل کسورات پرونده‌های بستری بیماران تحت پوشش بیمه تأمین اجتماعی در بیمارستان توانبخشی رفیده در سال 1400 اقتصاد سلامت Health Economics پژوهشی Original <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><span style="line-height:2;"><strong>هدف </strong>ازجمله مهم‌ترین موارد اتلاف منابع بیمارستان‌ها، کسورات بیمه‌ای هستند. کسورات بیمه‌ای نه‌تنها بیمارستان‌ها را با کمبود منابع مالی روبه‌رو می‌کنند، بلکه باعث ایجاد تنش بین سازمان‌های بیمه‌گر و &nbsp;بیمارستان‌ها می‌شوند. هدف از مطالعه حاضر، بررسی میزان و علل کسورات صورت‌حساب‌های بیماران تحت پوشش بیمه تأمین اجتماعی در بیمارستان توانبخشی رفیده بود.<br> <strong>روش بررسی</strong> مطالعه توصیفی ـ تحلیلی حاضر به‌صورت مقطعی در سال 1400 و در بیمارستان توانبخشی رفیده انجام پذیرفت. در این مطالعه، کلیه صورت‌حساب‌های مربوط به خدمات بستری ارسالی به سازمان بیمه تأمین اجتماعی موردبررسی قرار گرفتند. جهت جمع‌آوری داده‌ها از صورت‌حساب‌های ارسالی به بیمه تأمین اجتماعی، پرونده‌های بیماران و پرسش‌نامه محمدخانی استفاده شد. حجم نمونه شامل 776 پرونده بیماران بستری بود. برای آنالیز داده‌ها، از فراوانی و درصد و آزمون ضریب همبستگی اسپیرمن در نرم‌افزار SPSS نسخه 22 استفاده شد.<br> <strong>یافته‌ها</strong> بر اساس یافته‌های پژوهش، بیشترین فراوانی نسخ بستری مربوط به ماه آبان (7/48 درصد) بود. بیشترین کسور صورت‌حساب‌های بستری‌شدگان به‌ترتیب مربوط به دارو و لوازم مصرفی و حق‌العمل جراح و بیشترین علت آن اضافه درخواستی و کدگذاری غلط بود. در پاسخ به فرضیه های تحقیق، مستندسازی پرستاران و پزشکان با میزان کسورات ارتباط معنی‌دار داشت (p<0/0001).<br> <strong>نتیجه‌گیری </strong>برنامه‌ریزی برای کاهش کسورات بیمه‌ای نیازمند توجه به عوامل متعدد است. با آموزش قوانین بیمه و کتاب تعرفه به گروه‌های پزشکی و پرستاری، الکترونیکی کردن ارسال اسناد به بیمه تأمین اجتماعی و ایجاد مکانیزم‌های هشداردهنده در آن، استفاده از اهرم‌های نظارتی و شناسایی پزشکان و پرستاران مرتبط جهت ارائه بازخورد، نظارت بر عملکرد پزشکان و بررسی خطاهای مربوطه، ایجاد مکانیزم‌های تنبیه و تشویق، تعامل مستمر با بیمه تأمین اجتماعی جهت توجیه مدیران سازمان در خصوص نحوه ارائه خدمت به بیماران توانبخشی و علت طولانی شدن زمان بستری این گونه بیماران و همچنین اطلاع از تغییرات جدید در مجموعه قوانین بیمه تأمین اجتماعی می‌توان از بسیاری کسورات جلوگیری کرد.</span></span></span></div> <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><span style="line-height:2;"><strong>Objective </strong>Insurance deductions are among the most important causes&nbsp;of hospital resource waste.&nbsp;Insurance deductions cause financial problems for hospitals and create tension between insurance organizations and hospitals. This study, done by&nbsp;the University of Social Welfare and Rehabilitation Sciences,&nbsp;investigates the reasons for deductions of inpatient&nbsp;bills covered by Social Security Insurance in&nbsp;Rofeideh&nbsp;Rehabilitation Hospital<br> <strong>Materials & Methods</strong> The study&nbsp;was conducted cross-sectionally on 776 cases admitted to Rofeidah Rehabilitation Hospital in 2021. All invoices for inpatient services sent to Social Security Insurance&nbsp;were reviewed using the census method. To collect data, invoices sent to Social Security, inpatient records and checklists, and a 31-question questionnaire of Mohammadkhani et al. were used. The collected data were analyzed by descriptive (frequency and percentage) and analytical statistics (the Spearman correlation coefficient) in SPSS software, version 23.<br> <strong>Results </strong>According to the findings, the highest frequency of inpatient prescriptions was related to November 2021 (7.48%). The most frequent deductions of inpatients&rsquo; bills were related to medicine and consumables to the amount of 310815448 Rails and surgeon&rsquo;s fees in 187728448 Rails. Also, the most common reasons were requesting a surcharge and wrong coding. In this study, there was a significant relationship between the documentation of nurses and doctors and the amount of deductions (P<0.0001).<br> <strong>Conclusion </strong>Multiple causes affect the reduction of insurance deductions.
