<p style="text-align: justify;"><strong>هدف</strong> یکی از درمانهایی که اخیراً برای کودکان ناشنوا مطرح شده، کاشت حلزون شنوایی است. کاشت حلزون فناوری جدیدی در تجهیزات شنوایی و روش درمانی پذیرفتهشدهای برای کودکان کمشنوا محسوب میشود. در سالهای اخیر پژوهشگران به نقش خانواده و رابطه آن با کسب مهارتهای رشدی در کودکان مبتلا به آسیب شنوایی توجه ویژه مبذول داشتهاند. تجربیات بالینی نیز نشان داده است که عملکرد عاطفی و مشکلات روانشناختی مراقب اصلی، اثر نامطلوبی بر مهارتهای رشدی و تحولی کودکان کاشت حلزونشده خواهد داشت. پژوهش حاضر با هدف تعیین رابطه استرس والد-کودک با مهارتهای رشدی و تحولی کودکان کاشت حلزونشده انجام شدهاست. <br>
<strong>روش بررسی</strong> پژوهش حاضر مطالعهای پیمایشی از نوع همبستگی بود که در سال 1393 انجام شد. جامعه آماری این تحقیق را 268 نفر از کودکانی تشکیل میدادند که در بیمارستان الزهرای اصفهان عمل جراحی کاشت حلزون روی آنها انجام شده بود. از این جامعه 57 کودک (34 پسر و 23 دختر) که حداقل دو سال از عمل جراحی آنها گذشته بود، به روش تصادفی انتخاب و بررسی شدند. برای سنجش متغیرهای پژوهش از شاخص تنیدگی والدینی، مقیاس عملکرد بینالمللی لایتر، مقیاس رشد اجتماعی وایلند، مقیاس رشد و تحول زبان و مقیاس عملکرد ارتباطی کودکان کاشت حلزونشده استفاده شد. دادههای پژوهش با استفاده از نسخه 22 نرمافزار SPSS آمار توصیفی، روش همبستگی و تحلیل رگرسیون تجزیهوتحلیل شد.<br>
<strong>یافتهها</strong> نتایج حاصل از تحلیل دادهها نشان داد بین استرس والد-کودک با رشد زبانی (59/0-)، اجتماعی (38/0-) و ارتباطی (54/0-)کودک رابطه منفی (معکوس) معنیداری وجود دارد (01/0P≥). با توجه به نتایج تحلیل رگرسیون در تبیین رشد زبانی (متغیر ملاک) کودک براساس استرس والد-کودک (متغیر پیشبین) ملاحظه میشود که ضریب تبیین برابر 34/0=R2 است و نتایج بهدستآمده نشان میدهد که 34 درصد واریانس متغیر وابسته (رشد زبانی) از طریق استرس والد-کودک تعیین میشود. همچنین در تبیین رشد اجتماعی کودک (متغیر پیشبین) براساس استرس والد-کودک (متغیر ملاک) ملاحظه میشود که ضریب تبیین برابر 14/0=R2 است و نتایج بهدستآمده نشان میدهد 14 درصد واریانس متغیر وابسته (رشد اجتماعی) از طریق استرس والد-کودک تعیین میشود. علاوهبراین در تبیین عملکرد ارتباطی کودک (متغیر پیشبین) براساس استرس والد-کودک (متغیر ملاک) مشاهده میشود که ضریب تبیین برابر 29/0=R2 است و نتایج بهدستآمده از این امر حکایت دارد که 29 درصد واریانس متغیر وابسته (رشد اجتماعی) از طریق استرس والد-کودک تعیین میشود.<br>
<strong>نتیجهگیری</strong> درمجموع یافتههای این پژوهش نشان داد که استرس والد-کودک، بر بخش قابلتوجهی از مهارتهای رشدی و تحولی کودکان کاشت حلزونشده تأثیر میگذارد. با توجه به اهمیت و ضرورت رشدی و تحولی در کودکان کاشت حلزونشده، نتایج این پژوهش ضرورت ارائه خدمات مشاورهای و حمایتهای روانشناختی را برای این گروه از والدین گوشزد میکند. برایناساس میتوان برنامههایی را در قالب کارگاههای آموزشی برای مادران این کودکان پیشنهاد کرد. همچنین به روانشناسان و مشاوران خانواده و متخصصان خانواده درمانی توصیه میشود که در برنامههای درمانی و آموزشی خود به نقش فشار روانی در روابط والد-کودک توجه ویژهای داشته باشند.</p>
<p style="text-align: justify;"><strong>Objective</strong> One of the treatments that have been proposed recently for deaf children is cochlear implant. It is a new technology and an accepted treatment for children with hearing impairment. In recent years, researchers have paid special attention to the family and its relationship with the developmental skills of children with hearing loss. Clinical experience has also shown that emotional performance and psychological problems of the caregiver can have unfavorable effects on cochlear implanted children’s developmental skills. Therefore, the present study was conducted with the aim of determining the relationship of parent-child stress with developmental skills of children with cochlear implants.<br>
<strong>Materials & Methods</strong> The present study was a correlational study that was conducted in 2015. The study population comprised all children undergone cochlear implant surgery in Al-Zahra Hospital of Isfahan. Out of them, 57 participants (34 boys and 23 girls) were selected randomly. To evaluate the research variables, the Parenting Stress Index, Leiter International Performance Scale, the Vineland Social Maturity Scale, the Language Development Scale, and the Cochlear Implanted Children’s Communication Performance Scale were used. The data of the research were analyzed using descriptive statistics as well as correlational and regression analysis methods. SPSS version 22 was used for data analysis.<br>
<strong>Results</strong> The results of analyzing data indicated that there is a negative (reverse) significant correlation between parent-child stress and language development (-0.59), social development (-0.38), and communication development (-0.54) of children (P≥0.01). According to regression analysis investigating the explanation of language development (criterion variable) based on parent-child stress (predictive variables), the coefficient of determination equals to 0.34 (R2=0.34) and thus, 34% of the variance in the dependent variable (language development) are explained via parent-child stress. Also, in explaining the social development (criterion variable) based on parent-child stress (predictive variables), the coefficient of determination equals to 0.14 (R2=0.14) and thus, 14% of the variance in the dependent variable (social development) are explained via parent-child stress. Also, in explaining communication function (criterion variable) based on parent-child stress (predictive variables), the coefficient of determination equals to 0.29 (R2=0.29) and thus, 29% of the variance in the dependent variable (communication function) are explained via parent-child stress. <br>
<strong>Conclusion</strong> The results of this study showed that parent-child stress had a significant effect on developmental skills of children with cochlear implants. Due to the importance of developmental skills in children with cochlear implants, the results of this study warned the need to provide counseling and psychological support for this group of parents. Accordingly, the programs should be offered in the form of workshops for mothers of these children. Also, psychologists and family counselors and experts in family therapy are recommended to have special attention to the role of stress in parent-child relationships in treatment programs and training.</p>
