توانایی آزمونهای تحملی در پیشآگهی مزمنشدن علائم در بیماران مبتلا به اولین تجربه کمردرد حاد غیراختصاصی

<p style="text-align: justify"> <strong> هدف: </strong>علیرغم ارزیابی تحمل عضلات تنه در بیماران دچار کمردرد مزمن، این کار در بیماران حاد یا انجام نشده و یا در بیمارانی انجام شده که چندین تجربه کمردرد حاد داشتهاند. هدف از این تحقیق مقایسه میزان تحمل عضلات فلکسور و اکستانسور تنه در بیماران مبتلا به اولین تجربه کمردرد با افراد سالم و همچنین امکان پیشبینی طولانی شدن علائم بوده است.</p>
<p style="text-align: justify"> <strong> روش بررسی: </strong>در مرحله اول این تحقیق، طی یک مطالعه تحلیلی مقطعی از نوع مورد – شاهدی 30 بیمار دچار اولین تجربه کمردرد حاد که با استفاده از نمونهگیری غیر تصافی ساده انتخاب شده بودند، با 51 فرد سالم که بر اساس همتاسازی و جورکردن با بیماران انتخاب شده بودند، از نظر متغیرهای مورد بررسی مقایسه شدند. از آزمونهای تحمل اکستانسوری و فلکسوری ایتو برای ارزیابی تحمل عضلات تنه استفاده گردید و برای بررسی بهبودی از متغیرهای درد و ناتوانی استفاده شد.در مرحله دوم تحقیق، طی یک مطالعه تحلیلی طولی و آیندهنگر، بیماران تا شش هفته پیگیری شده و به دو گروه بهبودیافته و بهبود نیافته تقسیم شده و توانایی آزمونهای انجام شده بر روی آنان، در پیش آگهی بهبودی یا مزمن شدن درد در آنان بررسی گردید. جهت تجزیه و تحلیل دادهها از آزمونهای آماری کولموگراف–اسمیرنوف، تی مستقل و مجذور خی استفاده شد.</p>
<p style="text-align: justify"> <strong> یافتهها: </strong>نتایج آزمونهای تی مستقل نشان دهنده تحمل عضلانی پایینتر اکستانسوری و فلکسوری در گروه بیماران نسبت به افراد سالم بود (P<0/0001). بعلاوه تحمل فلکسوری بیمارانی که پس از شش هفته بهبود نیافتند، از ابتدا کمتر از بیماران بهبود یافته بود (P<0/05).</p>
<p style="text-align: justify"> <strong> نتیجه گیری: </strong>به نظر میرسد که بیماران کمردرد در اولین تجربه خود دچار کاهش تحمل شده و برای کاهش تحمل نیاز به تکرار دفعات حمله نباشد. همچنین آزمونهای تحملی میتوانند در پیشآگهی ناتوانی و مزمن شدن کمردرد و تفکیک بیمارانی که در معرض این رخداد هستند، بعنوان روشی غیر تهاجمی و کم هزینه نقش ارزندهای داشته باشد.</p>
<p dir="ltr" style="text-align: justify"><strong>Objective:</strong> Despite evaluation of spinal muscle endurance was done in chronic low back pain, this has not been studied in first episode acute low back pain. The purpose of present study was comparatively evaluating patients with first-episode low back pain and healthy subjects to predict disability and chronicity after six weeks with spinal muscle endurance tests.</p>
<p dir="ltr" style="text-align: justify"><strong>Materials & Methods:</strong> Through an analytical, cross sectional and case – control study in the first stage of research Eliot’s spinal endurance tests were used to evaluate endurance. Disability and pain were used to assess recovery. The study was conducted in two groups of patients (n=32) and normal subjects (n=51) using non-randomized simple sampling. In the second stage, a longitudinal prospective study was done. Studied variables were compared in recovered and non recovered patients after six weeks. Data were analyzed by using Kolmogorof – Smirnoff test, independent T test and Chi-Square.</p>
<p dir="ltr" style="text-align: justify"><strong>Results:</strong> The results of independent t tests showed lower muscular endurance in patients compared to normal individuals (P<0.0001). Also the results of independent t-tests showed lower flexor endurance in unrecovered patients (P<0.04).</p>
<p dir="ltr" style="text-align: justify"><strong>Conclusion:</strong> It seems low back pain patients can contract decreased endurance in the first episode and no need to repeat episodes. Results show may be endurance tests could differentiate acute low back pain patients with high risk to chronicity and disability.</p>
