بررسی میزان نیروی گیرش جانبی شست مددجویان کوادریپلژی که از تحریک الکتریکی عضلات استفاده مینمایند با استفاده از شبیهسازی رایانهای یک مدل استاتیکیسهبعدی از دست

<p style="text-align: justify"><strong>هدف:</strong> مفصل انتهایی شست دست مددجویان کوادریپلژی که از تحریک الکتریکی عضلات جهت بازیابی حرکت انگشتان استفاده مینماید، هنگام عمل گیرش جانبی دچار خمش بیشازاندازه و در نتیجه گیرش ناموفق میگردد. در فعالیتهای روزمره این نوع گیرش جهت گرفتن و جابهجاکردن برخی از اجسام کاربرد فراوانی دارد؛ لذا لازم است برای این دسته از مددجویان این امکان حرکتی بازیابی گردد. در حال حاضر، مشکل فوق توسط تثبیت بند انتهایی شست در امتداد بند میانی که نیاز به عمل جراحی دارد و منجر به ازدستدادن یک درجه آزادی میشود، رقع میگردد. در این تحقیق با استفاده از شبیهسازی رایانهای یک مدل استاتیکی از دست، زاویه و نیروی گیرش در روش متداول و روشهای پیشنهادی جایگزین محاسبه و مقایسه گردید.</p>
<p style="text-align: justify"><strong>روش بررسی:</strong> در مدل ارائهشده انگشتان، کف دست و جسم مورد گیرش بهصورت صلب، ثابت و عمود بر صفحه افق و شست بهشکل زنجیرهای از سه جسم صلب که توسط یک مفصل دو درجه آزادی به دست و دو مفصل یک درجه آزادی به یکدیگر متصل شدهاند، در نظر گرفته شد. به هر مفصل گشتاور مقاومتی، گشتاور فعال عضلات و گشتاور حاصل از نیروی خارجی بر بند انتهایی شست اعمال گردید. عضلات فلکسور بلند شست (FPL: Flexor Pollicis Longus)، اکستانسور بلند شست (EPL: Extensor Pollicis Longus)، جمعکننده شست (ADP: Adductor Pollicis) و دورکننده شست (APB: Abductor Pollicis Brevis) در مدل بهکاررفته شش روش متفاوت مورد بررسی قرار گرفت: 1- تحریک فلکسور بلند شست و فیوزکردن مفصل انتهایی شست در زاویه صفر (روش متداول)؛ 2- در زاویه 15؛ 3- در زاویه 30 درجه؛ 4- تحریک فلکسور بلند شست؛ 5- تحریک همزمان فلکسور بلند شست و EPL؛ 6- تحریک همزمان فلکسور بلند شست، ADP و APB بدون فیوز نمودن مفصل.</p>
<p style="text-align: justify"><strong>یافتهها:</strong> مقایسه مابین نتایج تجربی دیگران و نتایج حاصل از مدل، مؤید اعتبار مدل است. یافتهها آشکار نمود استفاده از عضله فلکسور بلند شست بدون فیوزنمودن مفصل انتهایی شست موجب گیرش جانبی نامناسب با نوک شست میگردد و فیوزنمودن مفصل در امتداد بند میانی، بدون ایجاد تغییر محسوسی در نیروی گیرش، زاویه گیرش را به شکل مؤثری بهبود میبخشد. اگرچه اضافهنمودن زاویه فیوز موجب افزایش ناچیز نیروی گیرش میشود، زاویه گیرش به مقدار قابل توجهی افرایش مییابد و حالت گیرش را نامناسب مینماید؛ لذا نتایج مدل مؤید اثربخشی روش متداول است. نتایج نشان میدهد میزان گشتاور خمشی خلفی آکستانسور بلند شست نمیتواند بر گشتاور خمشی قدامی فلکسور بلند شست غلبه نماید و تغییر قابل توجهی در حالت گیرش ایجاد نماید، لذا بهنظر میرسد این راه حل پیشنهادی موفقیتآمیز نباشد، اما در روش آخر، بدون نیاز به فیوزکردن مفصل، حالت گیرش تقریباً مشابه روش متداول و نیروی گیرش 60 درصد بیشتر از نیروی گیرش در روش متداول است.</p>
<p style="text-align: justify"><strong>نتیجهگیری:</strong> در استفاده از تحریک الکتریکی عضلات میتوان عمل گیرش جانبی شست را بدون عمل جراحی و با استفاده از تحریک الکتریکی همزمان عضلات فلکسور بلند شست، جمعکننده شست و دورکننده شست بازیابی نمود.</p>
<p dir="ltr" style="text-align: justify"><strong>Objective:</strong> In some applications of functional neuromuscular stimulation (FNS), the distal joint of the thumb (IP) in quadriplegic patients is sometimes surgically fused at zero degrees and the FPL is stimulated. This prevents hyperextension and extreme flexion of the IP joint during lateral pinch. However, IP joint fusion removes one degree of freedom from the thumb and may reduce the grip force. An alternative approach, preferably without surgical alterations, using sufficient electrical stimulation of selected muscles was investigated. A 3D model of prehensile lateral pinch was developed. Computer simulation of the model was used to find an approach providing the appropriate posture and adequate lateral grip force for quadriplegic patients using FNS.</p>
<p dir="ltr" style="text-align: justify"><strong>Materials & Methods:</strong> The model consists of a multi-rigid-body system connected by one or two degree(s) of freedom joints acted upon by passive resistive moments, active muscle moments and moments of external contact forces. Passive resistive moments were measured at each joint, active muscle moments were computed using a simple muscle model, and moments of external force were computed based on a force-displacement relationship for finger pads. In addition to the current strategy, two possible alternatives were studied: increasing the fused joint angle and activation of multiple muscles without joint fusion. Normal component of the grip force and its angle with respect to the horizontal plane were computed and compared for the studied cases.</p>
<p dir="ltr" style="text-align: justify"><strong>Results:</strong> Results showed, by using the current FNS strategy, a convenient posture and a grip force of 10.1 (N) are achieved which is comparable to what is measured experimentally and introduced in the literature. Increasing the joint fusion angle from 0 to 15 and 30 degrees in parallel with the activation of FPL increased the grip force from 10.1 to 10.7 and 11.2 (N), respectively, but resulted in inconvenient posture. Among all different combinations of the muscles with no joint fusion, only synchronous activation of ADP, APB, and FPL resulted in a convenient posture and a higher grip force of 16.6 (N).</p>
<p dir="ltr" style="text-align: justify"><strong>Conclusion:</strong> An alternative approach stimulating a combination of the named muscles without any need for surgical alterations might exist.</p>
