مداخلۀ زمینه ای طراحی شده برای اختلالات طیف اتیسم: پژوهش آزمایشی با استفاده از طرح تک موردی

<div style="text-align: justify;"><strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">اهداف:</span></span></strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> مطالعه حـاضر جهت بررسی میزان پذیرش مداخلۀ زمینـه ای طـراحی شده برای کـودکـان دارای اختـلال طیـف اتیسم (</span></span><span dir="LTR"><span style="font-size:12.0pt;">CI-ASD</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">) و اثربخشی احتمالی آن در توسعه مشارکت کودکان و خودکارآمدی مادران آنها اجرا شده است. این مداخله دارای سه مؤلفۀ اصلی است: هدایتگری والدین، الگوهای پردازش حسی کودک و حمایت اجتماعی از والدین</span></span><span dir="LTR"><span style="font-size:12.0pt;">.</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> جلسات مداخله شامل آموزش گروهی والدین و جلسات هدایتگری انفرادی بود، که با استفاده از بحث ها</span></span> <span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> و سوالات انعکاسی، به مادران کمک می شد، راهکارهایی برای دستیابی به اهداف درمانی خود بیابند. </span></span><br>
<strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">روش بررسی: </span></span></strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">این پژوهش آزمایشی، با استفاده از یک طرح تک موردی به روش </span></span><span dir="LTR"><span style="font-size:12.0pt;">A-B-A</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> برای سه خانواده مراجعه کننده به مرکز توانبخشی نوید عصر، در شهر تهران در بهار ۱۳۹۶ مورد اجرا قرار گرفته است. شرکت کنندگان از بین خانواده های کودکان دارای تشخیص</span></span><span dir="LTR"><span style="font-size:12.0pt;"> ASD </span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> بر اساس معیار های ورود (سن شناسنامه ای کودکان بین ۱۰-۳ سال</span></span><span style="font-family:times new roman,serif;"><span style="font-size:12.0pt;">؛ </span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">داشتن حداقل یک الگوی پردازش حسی ناهنجار؛ میزان تحصیلات مادران دیپلم یا بالاتر؛ تسلط کافی مادران در درک و بیان زبان فارسی) انتخاب شدند. این مطالعه در سه مرحله، به مدت هجده هفته اجرا شد.</span></span> <span style="font-family:b nazanin;"><span style="font-size:12.0pt;">در دوره خط پایه (مرحله اول) شرکت کنندگان به مدت چهار هفته مداخلات درمانی معمول </span></span><span dir="LTR"><span style="font-size:12.0pt;"> (TAU)</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">خود را دریافت داشته اند. دردوره مداخله (مرحله دوم) شرکت کنندگان به مدت ۱۰ هفته (آموزش گروهی والدین دو جلسه در ۱ هفته و ۹ جلسه هدایتگری در ۹ هفته) مداخلۀ زمینه ای طراحی شده برای کودکان دارای اختلالات طیف اتیسم</span></span><span dir="LTR"><span style="font-size:12.0pt;">(CI-ASD) </span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> را در کنار مداخلات درمانی معمول </span></span><span dir="LTR"><span style="font-size:12.0pt;"> (TAU)</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">دریافت نمودند.</span></span> <span style="font-family:b nazanin;"><span style="font-size:12.0pt;">در دوره پیگیری (مرحله سوم) شرکت کنندگان به مدت چهار هفته فقط مداخلات درمانی معمول (</span></span><span dir="LTR"><span style="font-size:12.0pt;">TAU</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">) خود را دریافت داشته اند. مداخلۀ زمینه ای توسط کاردرمانگری با ۲۰ سال سابقۀ کار در حیطۀ توانبخشی کودکان انجام شد.</span></span> <span style="font-family:b nazanin;"><span style="font-size:12.0pt;">فرم های ارزیابی در چهار مقطع زمانی: در شروع مطالعه قبل از مرحلۀ اول، پس از مرحلۀ اول، پس از مرحلۀ دوم و در پایان مرحلۀ سوم توسط مادران شرکت کننـده در مطالعـه تکمیـل شـد. نتایج درمان با استفاده از اندازه گیری عملکرد کاری کانادایی و مقیاس اکتساب هدف (برای مشارکت کودکان) و اندازه گیری خودکارآمدی والدین (برای احساس خودکارآمدی مادران) ارزیابی شد. یک مصاحبه نیمه ساختار یافته پس از پـایـان مرحله دوم (پـایـان مداخلۀ زمینه ای) به منظور بررسی سطح پذیرش درمان و تجربۀ شرکت کنندگان از مداخلۀ زمینه ای </span></span><span dir="LTR"><span style="font-size:12.0pt;">(CI-ASD)</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> به انجام رسید. داده ها به صورت</span></span> <span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> جدول، نمودار و تحلیل دیداری از مراحل مختلف پژوهش گزارش شده است. </span></span><span dir="LTR"><span style="font-size:12.0pt;"></span></span><br>
<strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">یافته: </span></span></strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">مقایسه نتایج پیش از مداخله و پس از مداخله نشان می دهد که دو شرکت کنندۀ باقی مانده در مطالعه، از نظر متغیرهای نتیجه: عملکرد کاری کودکان (۲۶، ۲۲، ۱۴، ۱۳) و (۲۹، ۲۳، ۱۲، ۱۲)، رضایت از عملکرد کاری کودکان (۲۵، ۲۲، ۱۷، ۱۳) و (۲۹، ۲۳، ۷، ۷)، دستیابی به اهداف (۶+، ۲+، ۶-، ۶-) و (۶+، ۲+، ۶-، ۶-) و احساس خودکـارآمـدی والدین (۵۷، ۵۷، ۵۳، ۵۰) و (۵۵، ۴۹، ۴۳، ۴۴) بهبود قابل ملاحظه ای داشته اند و یافته های مرحلۀ پیگیری نشان میدهند که روند بهبود نتایج تا ۴ هفته بعد از مداخلۀ زمینه ای ادامه داشته است. سطح پذیرش مادران از نحوۀ اجرا و نتایج مداخلۀ زمینه ای (</span></span><span dir="LTR"><span style="font-size:12.0pt;">CI-ASD</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">) توسط دو شرکت کنندۀ باقی مانده در مطالعه، بالا گزارش شده است. یکی از شرکت کنندگان احتمالاً به علت عدم درک و پذیرش این رویکرد درمانی، بعداز ۳ جلسه هدایتگری از ادامه درمان انصراف داد.</span></span><br>
<strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;">نتیجه گیری:</span></span></strong><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> دستآوردهای این پژوهش آزمایشی نشان می دهد که بر اساس نظر دو شرکت کنندۀ باقی مانده در مطالعه، سطح پذیرش مادران از نحوۀ اجرا و نتایج مداخلۀ زمینه ای </span></span> <span dir="LTR"><span style="font-size:12.0pt;">(CI-ASD)</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> بالا گزارش شده و احتمالاً می تواند یک مداخلۀ مؤثر برای کودکان با تشخیص </span></span><span dir="LTR"><span style="font-size:12.0pt;">ASD</span></span><span style="font-family:b nazanin;"><span style="font-size:12.0pt;"> و خانواده های آنها باشد. مطالعات آتی لازم است تا اثربخشی مداخلۀ زمینه ای طراحی شده برای کودکان دارای اختلالات طیف اتیسم را تأیید نماید و ویژگی خانواده هایی که می توانند از این مداخله بهره مند شوند، مورد بررسی قرار دهند.</span></span><span dir="LTR"><span style="font-size:12.0pt;"></span></span></div>
<div style="text-align: justify;"><strong>Objectives</strong>: The current study investigated the potential acceptability and effectiveness of Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD) in developing children’s participation and mothers’ <del cite="mailto:zahra%20pashazadeh" datetime="2019-07-09T16:17">parenting </del>self efficacy. Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD) involving contextually reflective occupational therapy combines 3 elements: parent coaching, child’s sensory processing patterns, and social support of parents. Intervention sessions comprised group parents training and (individualized) coaching sessions, which reflective questions and discussions with mothers support them in identifying best plans to achieve their therapeutic goals<span dir="RTL">.</span><br>
<strong>Method</strong>: In this pilot research, we employed a single subject, A-B-A interrupted time series design across 3 participants (3 mothers of children with the diagnosis of Autism Spectrum Disorders) reffered to Navid-e-asr rehabilitation center in Tehran, in spring of 2017. The participants were selected among families who have children with diagnosis of Autism Spectrum Disorders, ages 3-10 years and had at least one sensory pattern outside typical range based on the Short Sensory Profile II. Recruitment was based upon parent reports, Short Sensory Profile II data, demographic questionnaires and informed consents. The mothers’ education participated in this research were diploma or above diploma, and they could understand and speak Persian very well.<br>
We conducted the study for eighteen weeks in 3 phases. During baseline period or phase 1, the participants received treatments as usual (TAU) for 4 weeks. During intervention period or phase 2, the participants received Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD) for 10 weeks, 1 week group <del cite="mailto:zahra%20pashazadeh" datetime="2019-07-09T16:18">parents </del><ins cite="mailto:zahra%20pashazadeh" datetime="2019-07-09T16:18">mothers </ins>training (two sessions) and 9 weeks individualized coaching sessions (1 session per week), in addition to treatments as usual (TAU). During phase 3 or follow up, participants received only treatment as usual (TAU) for 4 weeks. The intervention therapist and coach was an occupational therapist, with 20 yrs. of clinical experience in pediatric rehabilitation implemented CI-ASD sessions during the study. In this pilot research, we administered outcome measures four times: prior to the study before phase 1 (pre- intervention<sub>1</sub>), after phase 1 (pre- intervention<sub>2</sub>), after phase 2 (post- intervention) and, after phase 3 (follow- up). We measured child participation by Canadian Occupational Performance Measure (COPM) and Goal Attainment Scaling (GAS), and parental self efficacy by Parent Self Efficacy Measure (PSEM). We conducted semi-structured interviews after completing the intervention period to explore acceptability of treatment and participants’ experiences of Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD). We used visual inspection of graphed data to identify variations between phases<span dir="RTL">. </span><br>
<strong>Results</strong>: Two of three participants <span dir="RTL">)</span>two mothers<span dir="RTL">(</span> completed the program and described growth in children occupational performance (COPM<sub>1</sub>) [13, 14, 22, 26] and [12, 12, 23, 29], satisfaction of occupational performance (COPM<sub>2</sub>) [13, 17, 22, 25] and [7, 7, 23, 29], goal attainment (GAS) [-6, -6, +2, +6] and [-6, -6, +2, +6], and parental self efficacy (PSEM) [50, 53, 57, 57] and [44, 43, 49, 55]. Two participants (mothers) found Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD) highly acceptable. Data showed development in occupational performance (COPM<sub>1</sub>), satisfaction of occupational performance (COPM<sub>2</sub>), goal attainment (GAS) and parental self efficacy (PSEM) continued after 4 weeks follow up. One of three participant (one mother) had difficulty accepting Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD) and left the study after 3 coaching sessions.<br>
<strong>Conclusion</strong>: On the report of two participants, Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD) was acceptable to deliver and may show promise as an intervention for children with the diagnosis of Autism Spectrum Disorders and their families. Further research is needed to determine the effectiveness of Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD). Since one family did not accept the Contextual Intervention adapted for Autism Spectrum Disorders (CI-ASD), it would be useful to identify characteristics of families who may benefit most from this intervention<span dir="RTL">.</span></div>
