بررسی تأثیر ارتزهای مچ پا-پایی بر متغیرهای راهرفتن و تعادل افراد سکته مغزی: مطالعه مروری

<p dir="RTL" style="text-align: justify;"><strong>هدف</strong> سکته مغزی به دنبال قطع یا اختلال خونرسانی به قسمتی از مغز ایجاد میشود. عوارض ناشی از سکته مغزی، میتواند از عوارض جزئی همانند علائم عصبی تا اختلالات خیلی شدید، بسته به محل و شدت آسیبدیدگی متفاوت باشد. همیپارزی یکی از شایعترین اختلالات در افراد دچار سکته مغزی در فاز حاد این عارضه است. برخی اختلالات دیگر نیز همچون اختلالات تعادلی وضعیتی نیز ممکن است به دنبال سکته مغزی مشاهده شود. تمامی اختلالات نورولوژیکی ناشی از سکته مغزی ممکن است سبب محدودیت دامنه حرکتی افراد از رنج کم تا زیاد شود. بازیابی توانایی راهرفتن یکی از مهمترین اهداف توانبخشی افراد دچار سکته مغزی است. از این رو استفاده از وسایل کمکی همچون ارتزها میتواند برای این افراد مفید واقع شود. یکی از متداولترین روشهای درمانی برای کنترل عارضه دراپفوت ناشی از سکته مغزی استفاده از ارتزهای مچ پا-پایی است. ارتز مچ پا-پایی با فراهم کردن ثبات مچ و قرار دادن پا در وضعیت مطلوب از افتادگی پا و سایر حرکات ناخواسته جلوگیری میکند و باعث میشود تا راهرفتن عادی شود. هدف مطالعه مروری حاضر ارزیابی تأثیرگذاری ارتزهای مچ پا-پایی بر تعادل و مؤلفههای زمانیمکانی، کینتیکی و کینماتیکی راهرفتن افراد با عارضه دراپفوت ناشی از سکته مغزی است.<br>
<strong>روش بررسی</strong> جستوجو در پایگاههای اطلاعاتی گوگل، اشپرینگر، ساینسدایرکت، پابمد، و اوید برای یافتن مطالعات در بازه زمانی ۱۹۹۶ تا ۲۰۱۶ که تاثیر ارتوزهای مچ پا- پایی را در افتادگی مچ سنجیدهاند، انجام شده است. پس از بررسی و طبقهبندی مقالات به دست آمده، مقالات موجود بر اساس متغیرهای زمانی مکانی راه رفتن، کینتیک، کینماتیک و تعادل تحلیل شدند. در نهایت ۲۱ مطالعه جهت ارزیابی نهایی انتخاب شدند.<br>
<strong>یافتهها</strong> ۲۱ مقاله درباره تأثیر ارتز مچ پا-پایی بر متغیرهای راهرفتن افراد دراپفوت ناشی از سکته مغزی تجزیهوتحلیل شده است. متغیر زمانیمکانی در ۱۴ مقاله و مؤلفههای کینتیک و کینماتیک در هفت مقاله ارزیابی شده است. ارتز مچ پا-پایی تأثیر معناداری بر طول و عرض گام داشته است، ولی بر سرعت، کادنس، قرینگی راهرفتن و تعادل افراد تأثیر معنادار نداشته است. همچنین استفاده از ارتز مچ-پایی سبب بهبود مؤلفههای کینتیکی راهرفتن، کینماتیک مچ پا و کینماتیک زانو در فاز ایستایی میشود، ولی بر کینماتیک مفصل زانو در فاز نوسانی و کینماتیک مفصل هیپ در صفحه فرونتال و ساجیتال تأثیر چندانی نداشته است. درباره تأثیر ارتز بر ممان اینرسی و پاور مفاصل مطالعات اندکی وجود داشت، ولی نتیجه حاکی از نبود تفاوت معنادار در این مؤلفهها بود.<br>
<strong>نتیجهگیری</strong> این مطالعه نشان داد مدل های مختلف ارتز مچ پا-پاییاستاتیک یا دینامیک، بسته به جنس و طراحیشان، تأثیرهای متناقضی بر تعادل، مؤلفههای کینتیک، کینماتیکی و متغیرهای راهرفتن افراد سکته مغزی میگذارند. استفاده از ارتزهای مچ پا-پایی در مقایسه با وضعیت بدون ارتز سبب راه رفتن مناسب تر افراد با عارضه افتادگی مچ پا میشود. با توجه به نتایج این مطالعه، بسته به نیاز و شرایط هر بیمار باید بهترین و مناسبترین ارتز مچ پا-پایی به صورت انحصاری و با در نظر گرفتن شرایط هر فرد برای او طراحی و ساخته شود. </p>
<p dir="LTR" style="text-align: justify;"><strong>Objective</strong> Stroke occurs when the supply of blood to the brain is either interrupted or reduced. The clinical presentation varies from minor neurological symptoms to severe deficits, depending on the location and the size of the brain lesion. Hemiparesis is one of the most striking features in the acute phase. Many other deficits may also be present, including postural imbalance. All persistent neurological deficits may cause more or less severe activity limitations in several domains of human functioning. Regaining walking ability is a major goal during the rehabilitation of stroke patients. Therefore, using orthoses can be beneficial for them. Ankle-Foot Orthoses (AFOs) is one of the most common therapeutic approaches to control foot drop among stroke patients. AFOs prevent drooping or other unintended movements of the foot and ankle by providing stability in optimum conditions. It also helps in regaining normal walking posture in stroke patients. The aim of this review was to evaluate the efficiency of the AFOs on balance and examine the effectiveness of temporal spatial and kinetic gait kinematics in stroke patients with foot drop.<br>
<strong>Materials & Methods</strong> Science Direct, Springer, Google Scholar, PubMed, Ovid databases were searched for articles published between 1996 and 2016 of studies on patients with drop foot wearing the AFOs. After reviewing and categorizing the articles, they were analyzed based on spatiotemporal parameters, gait kinetics, gait kinematics and stability. A total of 21 articles were selected for final evaluation.<br>
<strong>Results</strong> Twenty-one articles were analyzed in relation to the effect of the AFOs on gait parameters in stroke patients. Spatiotemporal parameters were evaluated in 14 articles, and kinetics and kinematics parameters were analyzed in seven articles. AFOs have a significant impact on the length and width of the steps but had no significant effect on speed, cadence, symmetry of gait and balance. Also, AFOs improved kinetic parameters of gait, ankle kinematics and kinematics of the knee in the static phase but had no significant effect on knee joint kinematics and kinematics of the hip joint in the frontal and sagittal. There are a few studies with regard to the effects of AFOs on the moment of inertia and joints power, but the results of the present study showed no significant difference in these parameters.<br>
<strong>Conclusion</strong> This study showed that the AFOs based on the models (static or dynamic) had a paradoxical effect on balance, kinetic and kinematic parameters of gait in the stroke patients. AFOs had a significant improvement in balance, kinetic and kinematic parameters of gait compared to those without orthosis situation in the stroke patients. According to the result of this study, depending on the patient's needs and situation, the best and the most suitable ankle foot orthoses should be designed and custom molded for them.</p>
