مطالعه اثر زانوبند نئوپرنی و بانداژکشی روی درد و حس وضعیت مفصلی در بیماران مبتلا به استئوارتریت زانو
Influence of Elastic Bandage and Neoprene Sleeve on Knee Position Sense and Pain in Subjects with Knee Osteoarthritis

<p style="text-align: justify"><strong>هدف:</strong> هدف از این پژوهش تعیین تأثیر زانوبند نئوپرنی روی حس وضعیت مفصلی و درد بیماران استئوارتریت یکطرفه زانو و مقایسه میزان تأثیر آنها بود. در یک مطالعه شبهتجربی 30 نفر (11 مرد، 19 زن) از افراد مبتلا به استئوارتریت یکطرفه زانو با سنین 75-33 سال مورد آزمایش قرار گرفتند. درد بیماران براساس VAS قبل از آزمایش میبایست cm2 از cm10 باشد.</p>
<p style="text-align: justify"><strong>روش بررسی:</strong> پای بیماران در حالت نشسته، بهطور فعال در زاویه بین 60-30 درجه قرار گرفت (زاویه هدف) و بعد از 5 ثانیه مکث با چشمان بسته و دستبهسینه این زاویه را چهار مرتبه بازسازی کردند. بازسازی اول جهت آشنایی بیمار بود. تفاضل میانگین سه دفعه بعد با زاویه هدف جهت مقایسه استفاده شد. برای تعیین میزان درد بیماران از معیار سنجش بصری (VAS) استفاده شد. (بدون درد=cm0 و درد شدید=cm10). یکی از مداخلهها بطور تصادفی بر روی پای بیماران بسته شد و مراحل فوق قبل و بعد از 20 دقیقه تکرار گردید . یک هفته بعد مداخله بعدی انجام شد.</p>
<p style="text-align: justify"><strong>یافتهها:</strong> با استفاده از آزمون تی زوجشده، علامتهای رتبهدار ویلکسون و ضریب پیرسون دادهها مورد تجزیه و تحلیل قرار گرفت و نشان داد که میانگین خطای زاویهای قبل و بعد از بستن زانو بند نئوپرنی تفاوت معنیداری دارد (P=0/037) و زانوبند حس وضعیت مفصلی را افزایش داده است، ولی بانداژ اثری روی حس وضعیت مفصلی نداشته (0/631=P). در مورد درد بیماران، هم بانداژ و هم زانوبند درد را بهطور معنیدار کاهش دادند (0/000=P) که این اثر در مورد زانوبند بیشتر بوده است (0/001=P).</p>
<p style="text-align: justify"><strong>نتیجهگیری:</strong> در این مطالعه میزان اثر زانوبند نئوپرنی بر حس وضعیت و درد بیماران مبتلا بیشتر از بانداژکشی بوده است. جهت تعیین تأثیر بانداژ پیشنهاد میشود که انواع دیگر بانداژ مورد بررسی قرار گیرند.</p>
<p dir="ltr" style="text-align: justify"><strong>Objective:</strong> to investigate whether a neoprene sleeve and elastic bandage around the knee joint of subjects with knee osteoarthritis (OA) would , in short term (a) reduce pain (b) improve knee joint position sense and comparison of their effect with each other if they have.</p>
<p dir="ltr" style="text-align: justify"><strong>Materials & Methods:</strong> In a semi-experimental study, 30 subjects (11 men, 19 women, age between 33-75) with unilateral knee OA. Subjects had to have at least 2cm from 10cm visual analogue scale (VAS) of knee pain for study entry.All patients were randomly assigned to either an elastic bandage or a neoprene sleeve. One week later they were assigned to the opposite selection. Joint position sense was assessed in the sitting position using an electrogoniometer and pain by VAS where 0cm equals no pain and 10 cm equals worst pain. Knee pain and JPS were assessed for each selection one week apart. During each visit assessment were performed at baseline and after 20 min of bandage/neoprene sleeve application.</p>
<p dir="ltr" style="text-align: justify"><strong>Results:</strong> the mean of scores for knee variables JPS and VAS was taken and paired-t test and Wilcoxon signed rank test was employed to calculate the different between two trails. Neoprene sleeve had significant effect on knee JPS (P=0.037). But elastic bandage had no effect (P=0.631). Both of them had significantly reduced knee pain. (P=0.000)</p>
<p dir="ltr" style="text-align: justify"><strong>Conclusion:</strong> In subjects with both neoprene sleeve and elastic bandage reduced knee pain with more effect of neoprene sleeve. Only the neoprene sleeve had effect on knee JPS.</p>
