</abstract_fa>
	<abstract>
Background: Shoulder impingement syndrome (SIS) is one of the most prevalent musculoskeletal disorders of the upper extremity. It is characterized by pain and impaired shoulder function, and may lead to reduced quality of life. Therapeutic exercise is considered one of the most effective non-surgical interventions for these patients and plays as a key factor in improvement of muscle strength, pain reduction, and restoration of shoulder function. Blood Flow Restriction (BFR) training is considered as a novel method that enables improvements in muscle strength and hypertrophy even in combination with low-load resistance training (LLRT) and may be a suitable option for patients with shoulder impingement syndrome. Despite its potential benefits, evidence regarding the effects of BFR training on muscle characteristics, pain, and functional outcomes in these patients remains limited.
Objective: the aim of this study is to investigate the effects of LLRT combined with BFR compared with LLRT alone on pain, muscle strength, muscle thickness, and shoulder-related disability in individuals with SIS.
Methods: This assessor-blinded, randomized controlled trial will include 28 adults diagnosed with SIS. Participants will be randomly allocated to either an LLRT+BFR group or an LLRT-only group. Both groups will receive 12 treatment sessions over four weeks, consisting of passive physiotherapy followed by progressive LLRT. The experimental group (LLRT+BFR) will perform exercises under 50% of limb occlusion pressure. Pain intensity (Numeric Pain Rating Scale) is the primary outcome. Secondary outcomes include pressure pain threshold, isometric strength of shoulder girdle muscles, muscle contraction ratio assessed by musculoskeletal ultrasound imaging, and shoulder pain and disability measured by the Shoulder Pain and Disability Index. All outcome measures will be assessed at baseline and within 72 hours after the final intervention.
Results: It is hypothesized that LLRT combined with BFR will result in greater reduction in pain and superior improvements in shoulder girdle muscle strength and contraction ratio compared with LLRT alone.
Conclusion: If the hypothesis comes true, this study will provide evidence regarding the clinical efficacy of adding BFR to LLRT in current shoulder rehabilitation protocols which already used in patients with SIS.
</abstract>
